What OS?
Adding the following to /etc/inittab on my RedHat 7.3 machine works for local hotsyncs:
P0:2345:respawn:/usr/local/bin/coldsync -s -t serial -dsync:3 -dmisc:1 -l
/var/log/coldsync -md ttyS1
